New patient registration

Use this service to register with the surgery.

We will be happy to see any friends or relatives staying with you for emergencies if they need to see a doctor, as temporary residents.

You can use this service if you:

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
  • your NHS number – help us to process your registration quickly by obtaining your NHS number from your previous GP practice
  • basic health and medical information
  • other personal details and demographics

You do not need proof of ID to register with a GP practice.

If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.
