We will be happy to see any friends or relatives staying with you for emergencies if they need to see a doctor, as temporary residents.
You can use this service if you:
- want to register at the surgery
- live within our catchment area
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- your NHS number – help us to process your registration quickly by obtaining your NHS number from your previous GP practice
- basic health and medical information
- other personal details and demographics
You do not need proof of ID to register with a GP practice.
If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.
